[OA] add staff error

Hello,

I have installed this for the first time. I believe I have everything
setup correctly, but I could be wrong. When I go to the start / end of
year, i try to add a teacher, I get the following error:

Staff Member Entry

Error: missing value for Table: staff Field: sal

Any ideas?
