[OA] Student Data Confirmation

Anyone used this function recently?  Somewhere in the last couple
upgrades it stopped working for me and I wonder if anyone else is having
the same problem?

When choosing this option for the admin page, the script runs to
completion but the PDF document is not created correctly.  When trying
to view it complains about being corrupt(rightly so I guess).

I noticed in the log file a couple pages of warnings followed by some
errors.  Attached is the last bit of the log if it'll help sort things
out.  Any ideas would be
