[OA] Student Enrolment & Withdrawal Problem

I have ran the meta.sql file but to no avail.
Initially  I ran the meta.sql file and it complained
of error of repetition since there was already a meta
table setup from the blank.sql.195 file that was used
to set up the openadmin database.
So I had to delete the initial meta table and run the
meta.sql file yet I still have the white screen of
death. I have installed all the dependencies but still
the same white screen of death is still displaying. 
This is what my error log file states:
